Below are examples of tile layouts that will appear on the donor wall of the Sarah & Edward Smith Health Sciences Center. Please complete the following registration form, indicating the text that you would like to have appear on your donor tile.
Each tile can contain up to three lines of text, with a maximum of 28 characters per line. Please note: spaces, periods and other punctuation count as characters.
